超声乳化人工晶状体植入联合房角分离术治疗闭角型青光眼 合并白内障的疗效观察

    目的 分析超声乳化人工晶状体植入联合房角分离术治疗闭角型青光眼合并白内障对患者视力水平、眼压的影响,为临床 治疗该疾病提供参考。方法 选取常熟市第一人民医院于 2018 年 12 月至 2021 年 12 月收治的闭角型青光眼合并白内障患者 80 例,以随 机数字表法分为对照组(40 例,采用超声乳化人工晶状体植入联合小梁分离术)和观察组(40 例,采用超声乳化人工晶状体植入联合房 角分离术),两组患者术后均随访 6 个月。比较两组患者临床疗效,术前、术后 1 个月的最佳矫正视力(BCVA)、眼压水平、中央前房 深度、角膜中心厚度、角膜内皮细胞平均面积、角膜内皮细胞密度及生活质量评分,以及术后并发症发生情况。结果 对照组患者临床 总有效率为 77.50%,观察组为 95.00%,观察组显著高于对照组;与术前比,术后 1 个月两组患者最小分辨角对数(logMAR)视力值、 眼压显著降低,观察组显著低于对照组;中央前房深度显著升高,观察组显著高于对照组;较术前,术后 1 个月两组患者角膜内皮细胞 平均面积均显著升高,但观察组显著低于对照组;角膜内皮细胞密度显著降低,但观察组显著高于对照组(均P<0.05),两组角膜中心 厚度组内、组间比较,差异无统计学意义(P>0.05);较术前,术后 1 个月两组生活质量评分均显著升高,观察组患者躯体功能、心理健 康、社会功能、情感职能、总体健康评分显著高于对照组;观察组术后并发症总发生率显著低于对照组(均P<0.05)。结论 超声乳化 人工晶状体植入联合房角分离术治疗有助于提高闭角型青光眼合并白内障患者的临床效果,降低眼压,改善视力水平,减轻角膜内皮功 能损伤,并减少并发症,提高生活质量。

    Abstract:

    Objective To analyze the effects of phacoemulsification and intraocular lens implantation combined with goniosynechialysis on visual acuity and intraocular pressure of patients with angle closure glaucoma with cataract, provide reference for clinical treatment of the disease. Methods A total of 80 cases angle closure glaucoma with cataract patients who were admitted to Changshu No.1 People's Hospital from December 2018 to December 2021 were selected and divided into the control group (40 cases, treated with phacoemulsification and intraocular lens implantation combined with trabeculectomy) and the observation group (40 cases, treated with phacoemulsification and intraocular lens implantation combined with goniosynechialysis) by the random number table method, patients in the two groups were all followed up for 6 months. The clinical efficacy,the best corrected visual acuity (BCVA), intraocular pressure, central anterior chamber depth, central corneal thickness, mean corneal endothelial cell area, corneal endothelial cell density and quality of life scores before and one month after operation, and postoperative complications occurrence of patients in the two groups were compared. Results The total clinical effective rate of patients in the control group and observation group were 77.50% and 95.00%, respectively, and the observation group was significantly higher than the control group; compared with before operation, the logarithm of the minimum angle of resolution (logMAR) value and intraocular pressure of patients in the two groups decreased significantly one month after operation, and the observation group was significantly lower than the control group; central anterior chamber depth of patients in the two groups increased significantly, and the observation group was significantly higher than the control group; compared with before operation, the mean corneal endothelial cell area of patients in the two groups increased significantly one month after surgery, but the observation group was significantly lower than the control group; the density of corneal endothelial cells of patients in the two groups decreased significantly, but the observation group was higher than the control group (P<0.05), there was no significant difference in the corneal central thickness between the two groups (P>0.05); compared with before operation, the quality of life scores of patients in the two groups increased significantly one month after operation, and the scores of physical function, mental health, social function, emotional function and overall health of patients in the observation group were significantly higher than those in the control group; the postoperative complications occurrence rate of patients in the observation group was significantly lower than that in the control group (P<0.05). Conclusion Phacoemulsification and intraocular lens implantation combined with goniosynechialysis can improve the clinical effect of angle closure glaucoma with cataract patients, reduce intraocular pressure, improve visual acuity, alleviate the corneal endothelial function damage, reduce complications, and improve the quality of life.
